Geffer Effervescent Granules is an over-the-counter medicine containing Metoclopramide. It is useful for treating the symptoms of hyperacidity (excess acid in the stomach), abnormal presence of air and gas in the stomach and digestive difficulties.
Geffer counteracts the symptoms of hyperacidity (heartburn and pain) even when accompanied by slow gastric transit (slow digestion). Reduces nausea, aerophagia and meteorism.

Hypersensitivity to any of the active substances or excipients. Patients with glaucoma, pheochromocytoma, epileptic disease, Parkinson's disease and other overt extrapyramidal disorders or undergoing therapy with anticholinergics. Cases in which stimulation of intestinal motility may prove dangerous, for example in the presence of gastrointestinal haemorrhage, perforation, mechanical obstruction. Patients with Porphyria. Children under 16 years of age. Pregnancy and breastfeeding.

Avoid concomitant administration of neuroleptics, phenothiazines, butyrophenones, thioxanthenes, etc., sedatives and alcohol. Neuroleptic malignant syndrome has been reported with metoclopramide both as monotherapy and in association with neuroleptics. In case of symptoms of neuroleptic malignant syndrome, metoclopramide must be discontinued immediately and appropriate treatment must be instituted. Use of the product may give rise to disturbances that alter the normal state of alertness; patients involved in driving vehicles or operating potentially dangerous machinery should take this into account. After a maximum of 3 days of treatment without appreciable results, consult your doctor. Elderly patients should not exceed 3 days of consecutive treatment and should stick to the minimum dosages indicated. Important information about some of the excipients. This medicine contains 288 mg of sodium per sachet, equivalent to 14.4% of the maximum daily intake recommended by the WHO which corresponds to 2 g of sodium per day for an adult. The sedative effects of metoclopramide are potentiated by alcohol. Anticholinergics and morphine derivatives antagonize the intestinal motility of metoclopramide. The sedative effects of CNS depressants (morphine derivatives, hypnotics, anxiolytics, sedative antihistamines, sedative antidepressants, barbiturates, etc.) and of metoclopramide are potentiated. The association of metoclopramide with drugs inducing extrapyramidal effects such as phenothiazines, butyrophenones and thioxanthenes should be avoided (in particular, the activity of phenothiazines is increased, independently of the appearance or potentiation of extrapyramidal effects). It increases the effects of MAOIs, sympathomimetics, and tricyclic antidepressants. Due to the prokinetic effect of metoclopramide, the absorption of some drugs may be altered. Metoclopramide may reduce the bioavailability of digoxin, while increasing the bioavailability of ciclosporin. It reduces the CNS effects of apomorphine. It reduces the bioavailability of cimetidine by an average of approximately 22%, without however having clinically relevant consequences. Metoclopramide interacts with serotonergic drugs (e.g. selective serotonin reuptake inhibitors), increasing the risk of serotonin syndrome.
SIDE EFFECTS

The following undesirable effects may occur following the use of this drug. With a frequency rare >=1/10,000, <1/1,000. Systemic disorders: tiredness. Endocrine disorders: hypertensive crises, sometimes with fatal outcome, have been reported in patients with pheochromocytoma; therefore, the use of this drug is contraindicated in these patients. Following the use of the product, in relation to the prolactin-promoting activity of metoclopramide, hyperprolactinemia, menstrual cycle disorders, galactorrhea and gynecomastia in men may occur. Nervous system disorders: drowsiness, tiredness, dizziness and extrapyramidal reactions of various types, usually dystonic, have been reported with the use of metoclopramide. These may include facial spasms, trismus, spasms of the extra-ocular muscles with oculogyric crises, abnormal head positions; these reactions usually regress within 24 hours of stopping treatment. In some elderly patients treated for prolonged periods, the development of tardive dyskinesia, potentially irreversible, has been reported; therefore, in elderly patients, therapies lasting more than 3 days should absolutely be avoided. Metabolism and nutrition disorders: cases of porphyria have been reported. Frequency not known (frequency cannot be estimated from the available data). Nervous system disorders: neuroleptic malignant syndrome. Vascular disorders: transient increase in blood pressure. Reporting of suspected adverse reactions.
