Do not exceed 2 puffs for each dosage and 64 puffs (4 puffs per hour over a 16-hour period) in any 24-hour period. Abrupt cessation of smoking For smokers who want and are ready to quit smoking immediately. Individuals must stop smoking completely while taking Nicorettequick. The following graph illustrates the recommended timing of use for the oromucosal spray during the complete treatment (phase I) and during the reduction phases (phase II and phase III). Phase I: weeks 1 to 6 Use 1 or 2 puffs when you would have smoked a cigarette or if you feel the desire to smoke. If a single puff does not control the urge to smoke within a few minutes, dispense a second puff. If 2 dispensings are required, it will be possible to dispense 2 consecutive dispensings in the future. Most smokers will need 1 or 2 puffs every 30 minutes to 1 hour. Phase II: weeks 7 to 9 Start reducing the number of daily dispensings. By the end of week 9, subjects must have HALVED the average number of daily puffs used in phase I. Phase III: weeks 10 to 12 Continue to reduce the number of puffs per day so that, in week 12, subjects are using no more than 4 puffs per day. When subjects have reduced the number of sprays to 2-4 per day, the use of the oromucosal spray should be stopped. Example: If you smoke 15 cigarettes per day on average, 1-2 puffs should be used at least 15 times throughout the day. As an aid to continuing smoking cessation after phase III, individuals may continue to use oromucosal spray in situations where they are strongly tempted to smoke. You can use a single puff in situations where you feel the urge to smoke, with a second puff if the first has had no effect within a few minutes. At this stage, no more than four sprays per day should be used. Gradual cessation through progressive reduction of smoking. For smokers who do not want or are not ready for an abrupt cessation of smoking. The oromucosal spray can be used between cigarettes in order to prolong smoke-free intervals and with the aim of reducing smoking as much as possible. The patient should be aware that inappropriate use of the spray can increase adverse effects. One cigarette must be replaced with one dose (1-2 puffs) and an attempt to quit smoking must be made as soon as the smoker feels ready and in any case no later than 12 weeks from the start of treatment. If a reduction in cigarette consumption is not achieved after 6 weeks of treatment, a healthcare professional should be consulted. After smoking cessation, the number of puffs per day should be gradually reduced. When subjects have reduced administration to 2-4 sprays per day, the oromucosal spray should be discontinued. Regular use of the oromucosal spray beyond 6 months is not recommended. Some former smokers may need longer treatment with oromucosal spray to avoid starting to smoke again. Residual oromucosal spray should be saved for use in the event of a sudden desire to smoke. Pediatric population Do not give Nicorettequick to anyone under 18 years of age. There is no experience in treating adolescents under 18 years of age with Nicorettequick. Method of administration After preparation, aim the spray nozzle as close to your mouth as possible. Firmly press the top of the dispenser and release a spray into your mouth, avoiding your lips. Subjects must not inhale while dispensing, to prevent the spray from being nebulized into the respiratory tract. For best results, do not swallow for a few seconds after dispensing. Subjects should not consume food or drink at the same time as taking the oromucosal spray.

CONSERVATION

Do not store at a temperature above 30 °C

WARNINGS

Nicorettequick should not be used by non-smokers. The benefits of quitting smoking outweigh any risks associated with the correct administration of nicotine replacement therapy (NRT). A risk-benefit assessment should be made by appropriate medical personnel, for patients with the following conditions: • Cardiovascular disorders: dependent smokers who have recently had a myocardial infarction, unstable or aggravated angina, including Prinzmetal's angina, serious cardiac arrhythmias or recent cerebrovascular accident and/or those suffering from uncontrolled hypertension, they must be encouraged to quit smoking through non-pharmacological interventions (e.g. counselling). If these fail, oromucosal spray may be considered but, as safety data in this patient group are limited, treatment should only be initiated under close medical supervision. • Diabetes mellitus. Patients with diabetes mellitus should be advised to monitor blood glucose levels more carefully than usual if smoking is stopped and NRT is initiated, as the reduction in nicotine-induced catecholamine release may affect carbohydrate metabolism. • Allergic reactions: predisposition to angioedema and urticaria. • Kidney damage and hepatic impairment: Use with caution in patients with moderate to severe hepatic impairment and/or severe renal impairment, as decreased clearance of nicotine or its metabolites may occur, with the possibility of increased adverse effects. • Pheochromocytoma and uncontrolled hyperthyroidism: use with caution in case of uncontrolled hyperthyroidism or pheochromocytoma, as nicotine causes the release of catecholamines. • Gastrointestinal pathology: nicotine can exacerbate symptoms in patients with esophagitis, gastric or peptic ulcers; NRT preparations should be used with caution under these conditions. Pediatric population Danger in children: doses of nicotine tolerated by smokers can cause serious toxicity in children, even with fatal outcomes. Products containing nicotine should not be left in places where they could be handled or ingested by children; see section 4.9 Overdose. Transmitted addiction: transmitted addiction can occur, but is less harmful and easier to stop than smoking addiction. Stopping Smoking: Polycyclic aromatic hydrocarbons in tobacco smoke induce the metabolism of drugs metabolized by CYP1A2 (and possibly CYP1A1). When a smoker stops smoking, he or she may experience a slowdown in metabolism, resulting in increased levels of these drugs in the blood. This is potentially more clinically relevant for products with a narrow therapeutic window, such as theophylline, tacrine, clozapine and ropinirole. With cessation of tobacco use, the plasma concentration of other medicinal products metabolised in part by CYP1A2 (e.g. imipramine, olanzapine, clomipramine and fluvoxamine) may increase, although data supporting this theory are scarce and the possible clinical significance of this effect for such medicinal products is unknown. A limited amount of data indicates that smoking may induce the metabolism of flecainide and pentazocine. Excipients: This medicine contains approximately 7 mg of alcohol (ethanol) per actuation, which is equivalent to 97 mg per mL. The quantity for one shot is equivalent to less than 2 mL of beer or 1 mL of wine. The small amount of alcohol in this medicine will not have any significant effect. This medicinal product contains less than 1 mmol (23 mg) sodium per spray, i.e. it is essentially 'sodium-free'. This medicinal product contains 11 mg of propylene glycol per actuation, equivalent to 150 mg/mL. Due to the presence of butylated hydroxytoluene, Nicorettequick may cause localized skin reactions (e.g. contact dermatitis) or irritation of the eyes and mucous membranes. Care should be taken to avoid contact of the oromucosal spray with the eyes during administration.

INTERACTIONS

No clinically significant interaction between nicotine replacement therapy and other drugs has yet been confirmed. These include emotional or cognitive effects such as dysphoria or depressed mood; insomnia; irritability, frustration, or anger; anxiety; difficulty concentrating and restlessness or impatience. Physical effects such as a decrease in heart rate may also occur; increased appetite or weight, dizziness or presyncopal symptoms, cough, constipation, bleeding gums or aphthous ulceration, or nasopharyngitis. Additionally, and of clinical importance, nicotine cravings may cause a profound urge to smoke. Nicorettequick may cause adverse reactions similar to those associated with nicotine taken in other ways and dependent primarily on dose. In susceptible individuals, allergic reactions such as angioedema, urticaria or anaphylaxis may occur. Local adverse effects of administration are similar to those observed with other forms taken orally. In the first days of treatment, mouth and throat irritation may occur; hiccups are also a particularly common effect. Habituation is normal with prolonged use. Daily data collection from study subjects demonstrated that the most common adverse events were reported with an onset in the first 2-3 weeks of oromucosal spray use, then decreasing. Adverse reactions observed with oromucosal nicotine formulations identified from clinical trials and during post-marketing experience are presented below. The frequency category was estimated from clinical studies for adverse reactions identified during post-marketing experience. Therefore, since to date no specific recommendations regarding the need for female contraception have been deemed necessary, the most prudent condition for women intending to become pregnant is not to smoke and not to use NRT. As regards men, it is known that smoking can cause adverse effects on male fertility, but there is no evidence of the need for special contraceptive measures by men during NRT treatment.Pregnancy Smoking during pregnancy is associated with risks such as intrauterine growth restriction, premature birth or stillbirth. Smoking cessation is the only truly effective intervention to improve the health of the pregnant smoker and her baby. The sooner abstinence is achieved, the better the outcomes will be. Nicotine is transmitted to the fetus, with consequences on the latter's respiratory movements and circulation. The effect on circulation depends on the dose. Consequently, pregnant smokers should always be advised to completely stop smoking without resorting to nicotine replacement therapy. The risk of continued smoking may pose more serious dangers to the fetus than the use of nicotine replacement products in a controlled tobacco cessation program. The use of Nicorettequick by a pregnant smoker should only be undertaken on the advice of a healthcare professional. Breastfeeding Nicotine passes freely into breast milk in quantities sufficient to affect the baby, even at therapeutic doses. Therefore, you should avoid taking Nicorettequick while breastfeeding. If you are unable to quit smoking, the use of Nicorettequick by breast-feeding smokers should only be undertaken on the advice of a healthcare professional. Women should take the product immediately after breastfeeding and wait as long as possible (2 hours is suggested) before the next feeding. Fertility Smoking increases the risk of infertility in women and men.
